What are the responsibilities and job description for the Clinical Navigator, RN - Pediatrics position at InterMed?
- Monitor and prioritize all telephone encounters utilizing clinical triage and telehealth principles via the pod clinical bucket (appropriately involving the provider as needed for guidance and resolution)
- Act as a liaison between the Central Intake department and the pod for clinical questions that arise; be available for warm transfers of complex patient calls when appropriate
- Answer incoming phone calls to the pod from Patient Service Representations, outside physicians, home health agencies, pharmacies, stat and critical imaging and lab results, etc.
- Act as the liaison between the provider and patient when delivery of treatment options and medical advice is required. Telephone encounter work returned from direct provider that requires home advice/treatment and
- Acts as a back-up to the Primary Clinical Assistant:
- Room patients
- Perform point of care testing as directed by provider
- Assist providers with clinical procedures
- Give injections and immunizations as directed
- Accurately document any vaccines given or historical vaccines (in ImmPact for pediatric vaccines)
- Prepare, stock and clean exam room(s), maintain supplies and equipment for treatment
- Oversight of clinical environment with attention to safety and infection control
- Oversight of clinical equipment including routine checks, calibration, cleaning and repairing
- Maintain emergency equipment
- Maintain pharmacy stock
- Oversight of vaccine storage, handling and supply
- Maintain confidentiality and compliance in all patient care activities
- Participate in continuing and professional development
- Complete InterMed clinical competence expectations.
- Be an informed clinical care resource
- Special projects as assigned by lead, coordinator or manager

JOB REQUIREMENTS
- Graduate from an accredited school of nursing, if RN. Unencumbered active registered nurse licensure in state of Maine, if RN or LPN.
- Or, successful completion of an accredited Medical Assistant program, preferably an Associate degree program; current CMA or RMA certification; or must obtain within 6 weeks of hire
- Professional appearance and demeanor
- Proficient in Windows applications, electronic medical record preferred
- Work collaboratively with all team members
- Demonstrate initiative, responsibility, flexibility and decision making
- Demonstrate critical and reflective thinking
- Ability to prioritize and handle multiple tasks
