Intermountain is a nationally recognized non-profit providing Hope & Healing to children, youth and families in need of improved mental and behavioral health. We are currently hiring a Case Manager located in Helena, MT. This position is the principal interface between the client and outside entities, providing effective leadership and advocacy in building effective relationships between treatment teams, families, and external stakeholders.

Salary: $18 - $20 per hour

Responsibilities:

  • Serves as primary liaison for families and external stakeholders. Works to resolve problems or situations that arise during treatment.
  • Educates families and external stakeholders about the treatment approach, philosophy and expectations for parent involvement.
  • In consultation with the team, parents and external stakeholders, documents and communicates sensitive and confidential information according to policy, in a professional and timely manner.
  • Ensures documentation of services such as consents, releases, intake procedures, referral contacts, placement, treatment, discharge reports and records are completed according to federal, state, and agency policy and procedures.
  • Compiles and charts case management activity.
  • Ensures financial and clinical eligibility for services prior to delivery of such services. Discusses and assists in making financial arrangements with parents and other agencies, makes referrals to financial resources, and procures funding commitments.
  • Ensures the Accounting/Payroll technician receives financial and clinical eligibility information. Completes billing for CM services within required timeframe.
  • Leads and coordinates all activities for referral (depending on the service area), admission, assessment, in-take, patient rights, child abuse and neglect reporting, and discharge in conformance with Intermountain policies and procedures.
  • Assumes responsibility for the overall design, implementation and documentation of treatment ensuring that the objectives are aligned with the treatment plan, treatment direction, and stage of treatment for the child and assuring that progress is being made.
  • Leads the treatment team and helps external entities identify and resolve external issues impacting treatment.
  • Coordinates with guardians, community agencies and professionals to ensure a cohesive approach throughout treatment is reported and discussed.
  • Takes lead in the coordination and facilitation of all meetings, in-take, staffing, and case conferences and ensures the treatment team thoroughly assesses plans, evaluates, reviews, and revises each child’s treatment direction and plan. Takes the lead in ensuring the treatment plans are reviewed within required time frames.
  • May serve as an agency representative during custody hearings, placement, or other judicial procedures.
  • In certain service areas, may serve as the medication representative communicating and coordinating as necessary with the contracted psychiatrist to ensure effective use of psychopharmacology in treatment.
  • Works with manager and team to identify and develop new referral sources and maintain and strengthen relationships with existing referral sources.
  • Attends in-service training, conferences, seminars, etc., to maintain knowledge of current trends and developments in the service delivery of treatment to children and their families as well as of information, standards, and protocols regarding Montana Medicaid/Mental Health Services Plan.
  • Monitors the day-to-day delivery of services, and takes the lead in collaborating with other providers to ensure the coordination of all treatment team efforts.
  • Completes and maintains in an accurate and timely manner a variety of records and reports in accordance with federal, state, and local laws, program policy, and Medicaid regulations.
  • Works with all teams in support of reaching strategic goals and revenue cycle management.
  • Works with manager to achieve balance of maximum occupancy with service efficacy in CM.
  • Seeks and refers parents to parent education and training toward skill development with techniques for child management, problem solving, practical everyday living, and increasing the sense of safety and nurturance within the parent/child relationship.
  • Discusses and coordinates transportation needs and medication management needs when appropriate.

OTHER DUTIES AND RESPONSIBILITES:

  • Works to support marketing and branding efforts.
  • Performs special projects or duties, assumes delegated responsibilities, and serves on ad hoc and other committees as assigned by their Manager or Supervisor.

Qualifications:

  • Bachelor’s degree in a social science field or unrelated field combined with relevant experience.
  • Case Management Certification preferred.
  • Montana driver’s license required.
