Job Description:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in various settings, including long-term care facilities and home community care. As an LPN, you will play a vital role in patient assessment, treatment, and education while working collaboratively with registered nurses and other healthcare professionals.
Responsibilities
- Administer medications and injections as prescribed by physicians.
- Perform gastric lavage and catheter care as required.
- Assist in discharge planning to ensure smooth transitions for patients returning home or to other care facilities.
- Provide care for patients with infectious diseases, ensuring adherence to safety protocols.
- Utilize electronic health record systems such as eClinicalWorks and Cerner for documentation and patient management.
- Participate in patient assessments and develop individualized care plans in collaboration with the healthcare team.
- Educate patients and their families on health management, medication administration, and post-discharge instructions.
- Maintain accurate records of patient progress and report any changes to the supervising nurse or physician.
Experience
- Valid LPN license in the state of practice.
- Experience in long-term care or home community care is preferred.
- Familiarity with discharge planning processes is advantageous.
- Proficiency in using electronic health record systems such as eClinicalWorks or Cerner is a plus.
- Previous experience administering injections and providing infectious disease care is desirable.
- Strong communication skills and the ability to work effectively within a multidisciplinary team are essential.
