What are the responsibilities and job description for the Case Manager position at JCDC?
Perform Purpose:
The Case Manager is responsible for identification of Medicaid waiver member (individual) needs, including physical health, mental health, social services, vocational services and long-term support services and the development of the member’s Individualized Service Plan of Treatment Plan to address those needs. The Case Manager is responsible for ensuring timely and coordinated access to an array of providers and services and developing solutions to address unique needs of individuals and coordinating services delivered as necessary and appropriate.
Duties/Responsibilities:
1) Ensure the health and safety of assigned individuals with disabilities at all times.
2) Perform face-to-face assessments with individuals to determine their interests and goals.
3) Obtain a thorough and accurate history including clinical, social, and environmental factors.
4) Identify each individual’s need for assistive/adaptive equipment.
5) Assess each individual’s clinical, behavioral, social, vocational and environmental information to assist the individual develop a treatment plan. This includes the individual’s interest and informed choice.
6) Acts as an advocate for the individual’s health care needs.
7) Serves as the Team leader on the Inter-disciplinary Team working together to assist the individual meets his/her goals.
8) Communicate plan objectives to appropriate individuals, agencies, care givers, or others as needed.
9) Identify individuals with potential for high risk complications and coordinate appropriate treatment in conjunction with the individual and the individuals required to assist the individual.
10) Establishes working relationships with referral sources and community resources.
11) Review benefit systems and conduct benefit analysis.
12) Evaluate quality of all services the individual is receiving.
13) Acquire data and evaluate necessary services for cost containment.
14) Document effectiveness of service coordination services.
15) Participates in Quality Improvement process.
16) Maintain strict individual confidentiality and comply with all HIPAA requirements.
17) Learn, understand and have working knowledge of the West Virginia Waiver manual. Stay abreast of any changes or potential changes in this manual.
18) This position requires the use of personal transportation in the execution of job responsibilities, included the periodic transportation of individuals.
