Two years experience in clinical or community resource settings; care coordination experience is desirable.

Must have experience in a healthcare clinical or community setting, experience in an outpatient setting is desirable. Must demonstrate a service-oriented approach and be a team player.
The Neurosurgical Patient Navigator works in collaboration and continuous partnership with chronically ill or high risk patients (Neuro-Oncology, Neurosurgery, and/or those with a diagnosis of a brain disease) who are not followed at a SKCC Multi-Disciplinary Clinic (MDC) but who are followed by the Vickie and Jack Farber Institute for Neuroscience at Jefferson in Center City.
S/he will coordinate care between diagnosis, treatment, survivorship or end of life. He/she participates in the assessment and ongoing support of patients in close collaboration with the the Care Team(s) s/he is supporting.
