SUMMARY OF POSITION RESPONSIBILITIES:

Under general supervision, performs social service functions to include assisting clients in meeting their bio-psycho-social requirements while enabling them to access through therapeutic treatment skills to achieve their optimal level of emotional health, including providing basic consultation, teaching & specialized clinical social services for patients / families receiving care. The clinical social services provided include: crisis intervention, individual therapy, short-term family therapy, group therapy & discharge planning. The MHP serves as an integral member of the treatment team & is also responsible for case management of patients internal/external support systems.

ESSENTIAL DUTIES:

1. Provide direct care to clients and may serve as primary counselor to specified caseload under clinical supervision;

2. Serve as resource person for other professionals and paraprofessionals in their specific area of expertise;

3. Attend and participate in individual care conferences, treatment planning activities and discharge planning;

4. Function as the client’s advocate in all treatment decisions affecting the client; and

5. Prepare and write notes or other documents related to recovery (e.g. assessment, progress notes, treatment plans, etc.).

6. Identifies Treatment Plan impairments based on assessment findings.

7. Provides age appropriate clinical interventions (individual, family, group therapy) using principles and practices as defined by assessment.

8. Attends patient treatment team meetings for assigned caseload.

9. Completes proper documentation in ShareNote following each treatment intervention.

10. Provides case management of caseload & communication with all sources to make sure quality of care.

11. Conducts intake interviews of patients and/or family members to gather needed information.

12. Makes psychosocial & diagnostic assessments & relates findings to necessary agency staff.

13. Counsels patients & other persons regarding diseases, treatments, social or financial situations, emotional stress and/or other problems.

14. Acts as a liaison between patients / families and the agency staff & other agencies.

15. Utilizes available information to evaluate the effectiveness of services provided in producing the desired outcomes & makes adjustments in patient care are indicated.

16. Assists in discharge planning & referrals to the community for ongoing services.

17. Maintains a current and unrestricted license with its respective professional board or licensing authority in the state of Louisiana

18. Serve as resource person for other professionals counseling persons with behavioral health disorders

19. Provide only those services that are within the person’s scope of practice

20. Serve as resource person for other professionals in their specific area of expertise;

21. Provides age appropriate clinical interventions (individual, family, group therapy) using principles & practices as defined by assessment.

POSITIONS SUPERVISED: None

LEVEL OF EDUCATION/TRAINING/QUALIFICATIONS:

1. Master’s Degree in Counseling/Social Work (or related field) & LCSW or LPC.

2. Two-years of qualifying clinical experience as an LMHP in the provision of services provided by the provider

3. Ability to apply the concepts of customer service in the workplace. Ability to use good professional judgment. Detail oriented. Ability to work independently, with minimal direct supervision.

4. Excellent organizational, prioritization, and interpersonal communication skills. Excellent oral and written communication skills

5. Ability to handle multiple tasks and meet critical deadlines. Ability to meet short and long-term goals and project deadlines.

6. Respect for the confidentiality of all communications required within job duties, including client/employee records and documents, and electronic transmission by voice, data, and wire.

7. Experience with multi-cultural/multi-lingual populations and multi-disciplinary teams.

8. Knowledge in mental health a skill in dealing with population.

9. Knowledge of individual, family & group therapy techniques, crisis intervention procedures, clinical assessment skills, therapeutic planning & techniques, principles & practices of social & psychological casework & social & welfare resources available in the community & hospital.

10. Knowledge of current social & economic conditions, trends and problems.

11. Analytical skills sufficiently strong to interpret situations and define the appropriate course of action.

12. Being able to explain technical data in a form understandable by patients, families, and agency staff.

13. Good interpersonal skills.

14. Skilled in organizing & prioritizing workloads to meet deadlines.

15. Possess a record void of negative reports with the Louisiana state nurse aid registry and the Louisiana direct service worker registry.

REQUIRED CERTIFICATION/LICENSE: Current License from the Louisiana State Board of Licensed Social Workers or Licensed Professional Counselor (LPC).

PHYSICAL DEMANDS: Position will require frequent standing, walking, sitting & talking. Must be able to work at times in stressful situations. Frequent exposure to communicable diseases.

WORKING CONDITIONS: The LMHP usually work in a community-based environment; however, an office setting may be utilized for some duties
