Jewish Family & Children's Service of Greater Boston invites you to apply for our position of Case Manager, Human Services Coordinator. 

HIGHLIGHTS 

  • Full-time Monday-Friday. Flexible schedule! Weekday shifts are typically 11-7, but can be a bit flexible. If interested in a Sunday shift, this can be discussed with supervisor upon hire in-person role at Brighton, MA location. 
  • $50,000 annual salary with $1,000 signing bonus paid after 90 days employment. 
  • Responsibilities include working with adults with disabilities on independent living skills in a community setting.   

ABOUT THE ROLE 

The Case Manager, Human Services Coordinator provides coaching and case management to adults with developmental disabilities, autism, and mental health needs. The Case Manager, Human Services Coordinator will partner with clients to set goals and develop strategies to meet those goals. The Case Manager, Human Services Coordinator will plan and facilitate weekend and other recreational activities, ensuring that participants maximize their community involvement. The Case Manager, Human Services Coordinator also run groups focused on social pragmatics and social connections. 

 Responsibilities of the Case Manager, Human Services Coordinator: 

  • Supporting the client to achieve independent living and social goals. 
  • Communicating effectively with the client and their family 
  • Providing coaching and skill building to clients in independent living skills.  
  • Managing client cases, including writing daily service logs, daily goal tracking, monthly progress notes, annual assessments, incident reporting, and updating files 
  • Collaborating with team to ensure consistent delivery of services. 
  • Attending required staff meetings, supervisions, and professional development 
  • Transporting JF&CS clients in your own vehicle as needed

ABOUT YOU 

Ideal candidates for the role of Case Manager, Human Services Coordinator bring a positive attitude, and a desire to create positive impacts on their clients lives by mentoring, coaching, and encouraging them to achieve their goals.

  • Organized and able to keep up with records and paperwork. 
  • A great verbal and written communicator and an empathic listener 

Requirements include:  

  • A bachelor's degree or at least 2 years relevant experience working with people with disabilities, working in mental health, childcare or education. 
  • A valid driver's license and own vehicle. Mileage is reimbursed. 
  • CPR and First Aid certifications (or ability to pass within the first 30 days of employment) 
  • Excellent written and verbal communication skills 
  • Familiarity with Microsoft Office suite (Outlook, Word, Excel)
