The Intake and Outreach Social Worker is responsible for outreach to hospitals, skilled nursing facilities, and community-based organizations, to educate potential clients and service providers about the benefits of the JFSLA Healthcare programs (Multipurpose Senior Services Program and Providence Community Care Coordination Program), with the goal of increasing client diversity. The Intake and Outreach Social Worker will work with potential clients to provide information, screening, and complete intake documentation for older adults and adults with disabilities discharged from hospitals, Skilled Nursing, or other community care facilities. This position requires an individual with strong networking, communication, and organizational skills.
Qualifications:

• Master’s Degree in Social Work or related field
• Bilingual Spanish is preferred
• Excellent networking, advocacy, and public speaking skills
• Knowledge of community resources for clients who are aging or who have disabilities
• Experience and interest in providing care to adults with disabilities and older adults
• Case management and community-based service delivery experience preferred
• Knowledge of community resources and the aging process
• Knowledge of safety issues when working with at-risk clients
• Experience conducting in-home visits is strongly preferred
• Ability to function independently and with flexibility
• Experience with Electronic Health Records systems desired
• Must be computer literate in MS Office applications (Word, Excel, and Outlook)
• Demonstrated ability to work in an interdisciplinary community setting maintaining appropriate professional boundaries
• Culturally sensitive and non-judgmental in working with a multicultural client population
• Must pass a criminal background check (Live Scan fingerprinting) prior to beginning employment

Responsibilities/Essential Functions:

• Create outreach quarterly plans in conjunction with program managers and effectively carry out plans in a timely manner
• Review outcomes from outreach efforts and monitor effectiveness of outreach plans, making changes as appropriate
• Conduct regular in-person outreach to hospitals, skilled nursing facilities, and community-based organizations, to educate potential clients and service providers about the benefits of the Healthcare Programs
• Lead regular Information sessions in person and via video conferencing platforms
• Create and maintain an Outreach and Resource Database
• Conduct the initial screening of applicants in person and on the phone
• Complete the intake process and provide information and assistance to potential clients
• Document outreach activities in the referral module of the Electronic Health Record System
• Other related duties as assigned
