JOB DESCRIPTION: JOB SUPPORT/COMMUNITY BASED SUPPORT STAFF
Reports to: Manager; Job Support/Community Based Supports
Primary Responsibilities: Provide support for individuals with disabilities within the community
Specific Responsibilities:
- Provide direct support and assistance to achieve their highest potential within the community
- Advocate for the individual with disabilities, including fostering of relationships within the community
- Monitor client's progress through frequent communication with the client's family and other staff
- Monitors client's safety within the community
- Report progress/and or concerns to the Supervisor as needed, via phone calls, e-mails or supervision meetings
- Review and respond to e-mails or text messages from Supervisor regarding scheduling and any other changes pertaining to the client
- Attend staff meetings/in-service, ( when held and if work schedule permits)
- Complete daily progress note documentation as required by JFCS and DDD ( Division of Developmental Disabilities). Forward daily progress note documentation to JFCS.
Qualifications:
- Bachelors degree in psychology, sociology, or related field or two years experience working with individuals with developmental disabilities
- 18 years or older
- Ability to communicate effectively with employers, employees, and other professional staff in a compassionate and professional manner
- Valid drivers license, registration, insurance, and good driving record
