RN Care Coordinator - Adult Day Program

Adult Day Health Care Program

Pay Range: $64,480-$81,120 Salary (Pay information will vary depending upon relevant experience for the position)

Generally works Monday-Friday days, occasional evenings

GENERAL SCOPE: Maintains the standards of nursing care and practice in the Day Services Department.

EDUCATION / QUALIFICATION: Current New York State Registered Professional Nursing license.

ESSENTIAL FUNCTIONS:

• Provides nursing care as necessary to meet the registrant’s care plan.
• Assesses the registrant and family response to program, nursing care and services.
• Works closely with the Director of Day Services, to maintain the staffing pattern as well as the short-term and daily scheduling time pattern for the program.
• Monitors the activities of nursing personnel to insure adherence to standards.
• All other duties as assigned

EXPERIENCE: Three (3) years clinical experience in a community nursing environment with UAS experience preferred. Acute care acceptable. Previous leadership responsibilities preferred.
