The Emergency Service Intensive Case Manager will work directly with homeless clients to create a professional case management plan and perform related duties as required to meet program guidelines.
Jewish Family Service Agency Emergency Services provides nondenominational, criteria based social resources to those who need assistance in these areas: Transportation, Emergency Assistance, Nutrition, and Housing with an emphasis on stability and self-sufficiency.
The Emergency Services Intensive Case Manager should be energetic, hardworking, and passionate about providing quality services to clients in hardship situations, including the homeless. This position is dependent on grant funding, which currently expires on September 30, 2021. While both our agency, and the grantor, are actively working on continued funding no guarantees can be made that additional funding for this position will be granted prior to September 30, 2021.
Tasks/Responsibilities
· Conduct client assessment to determine client needs and set up a case plan to be followed.
· Provide in home intensive case management to program eligible individuals
· Complete all paperwork in a timely manner related to and for clients being assisted.
· Case Manger will provide appropriate resources, tools, counseling to assist participants in achieving their case plan goals. Making referrals for services, following up, tracking and documenting participant’s progress.
· Maintain positive relations with clients, local community agencies and team members.
· Document all client contact, maintain client files, and document all information
· Be an active and collaborative member of the Emergency Services team at JFSA
· Participate in various committee meetings and trainings related to community resources.
· Maintain client confidentiality.
· Report client concerns/ needs to Director in a timely manner
· Maintain compliance with program guidelines.
· Perform other duties as assigned by the director and as needed for the programs.
Education and Experience
· Bachelor’s Degree in Social services or a related field
· 2 years of case management
· Experience working with the homeless
· Ability to communicate effectively verbally and in writing.
· Ability to work with individuals from various ethnic/cultural and socioeconomic backgrounds.
· Basic computer knowledge is required
· Possess a working knowledge of community resources within Clark County.
Skills/Requirements
· Dependable transportation and a current, valid, NV Driver License are required
· Ability to pass a background test And Drug screening
· Ability to work assigned schedule.
· Ability to be empathetic while maintaining appropriate boundaries
· Ability to work effectively and independently.
· Ability to climb stairs, walk, drive, lift, and carry up to 25 lbs daily
