Job Details

• Collaborate with staff to provide high-quality care, identify health concerns, and create personalized treatment plans.
• Foster awareness of the Chronic Care Model Processes to reduce health disparities in the community.
• Offer clinical consultation and supervision to fellow staff members and students.
• Ensure adherence to National Patient Safety Goals and participate in quality improvement activities.
• Attend mandatory in-services, drills, staff meetings, and regular age-specific educational trainings for professional development.
• Maintain a flexible schedule based on clinic needs, including evening and weekend rotations.
• Work across multiple locations in Lake County to broaden your experience and reach.

Skills You Need:

• Master's Degree in Nursing or postmaster certification as an APN.
• Appropriate certification by a National certifying body (ANCC, NCCC, NAPNAP, AMCB, or ACC) as required by state law.
• Current Illinois licensure as a Registered Nurse or Advanced Practice License.
