Job Details
We are seeking a highly motivated and experienced full-time Care Manager RN to join our dynamic medical team. As a Permanent Care Manager, you will be responsible for overseeing and managing the care of patients with chronic illnesses, including renal and kidney disease, in a variety of settings including home health, long-term care, hospice, and nephrology clinics. The ideal candidate will have a minimum of 2 years of experience in the medical industry, specifically in the areas of renal, kidney, and nephrology care.
Responsibilities:
* Provide comprehensive care management services to patients with chronic illnesses, including renal and kidney disease, in a variety of settings including home health, long-term care, hospice, and nephrology clinics.
* Develop and implement individualized care plans for patients based on their specific needs and goals.
* Collaborate with interdisciplinary teams, including physicians, nurses, social workers, and other healthcare professionals, to ensure the best possible outcomes for patients.
* Educate patients and their families on disease management, self-care techniques, and available resources.
* Monitor and evaluate patient progress, adjust care plans as needed, and document all patient interactions and interventions.
* Ensure compliance with all regulatory requirements and quality standards, including HIPAA and OSHA guidelines.
* Participate in quality improvement initiatives and other projects as assigned.
Qualifications:
* Active RN license in good standing with the state nursing board.
* Minimum of 2 years of experience in the medical industry, specifically in the areas of renal, kidney, and nephrology care.
* Bachelor's degree in nursing or related field preferred.
* Strong clinical assessment and critical thinking skills.
* Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
* Knowledge of home health, long-term care, hospice, and nephrology care practices and regulations.
* Proficiency in electronic medical records and other healthcare technologies.
* Ability to work independently and as part of a team in a fast-paced and dynamic environment.
