About this position:
The Community Health Nurse (CHN) is part of the Hospital’s Community Care Team (CCT). CCT is a program focused on improving the health of complex, chronically ill Howard County residents. This program helps individuals connect with resources that allow them to better manage their illness, improve their health and avoid preventable hospital encounters. CCT helps clients to meet their goals by providing health education, teaching chronic-disease management skills, and by connecting them to a medical home and other community resources. The CHN is an integral part of the multi-disciplinary team that includes Community Health Workers (CHW) and a Licensed Clinical Social Worker (LCSW). The CHN supports the implementation of a care plan and performs essential clinical functions as part of a client’s care team. This position works across care settings as well as conducts regular home visits.
Responsibilities include:
- Identifies, screens, engages and enrolls eligible patients in the acute and post-acute care settings and facilitates coordination of services pre and post discharge.
- Provides general health education as well as specific chronic disease management (e.g. teaching on blood pressure/blood glucose monitoring and medications). The CHN does not administer treatments or medications.
- Provides clinical recommendation to Community Health Workers (CHW) and day-to-day oversight of CHW caseload and activities.
- Follows-up with clients via phone calls, home visits and visits to other settings.
- Maintains knowledge about community resources appropriate to meet the social and medical needs of clients.

What you’ll bring:
Education and Experience:
- A Bachelor’s degree in Nursing from an accredited college or university; may consider candidates with a Bachelor’s degree in a related field and one year of experience as a Registered Nurse in lieu of BSN.
- A minimum of two years of experience in in public/community health as well as one year of additional experience in an administrative, supervisory, consultative or teaching capacity as a registered nurse.
- Experience working in a community setting or home setting preferred.
- Experience working as part of a multi-disciplinary team as well as with people of diverse backgrounds preferred.
Licenses & Certifications:
- Must maintain current Maryland License as an RN or licensure in a compact state that allows practice in Maryland.
- Must obtain CPR certification within sixty (60) days of hire and maintain thereafter.
- Must have valid driver’s license, clean driving record, and reliable transportation. This position may spend the majority of the time working away from the office.
