The Johns Hopkins Medicine Office of Population Health (OPH) is a Johns Hopkins Health System department comprised of management and patient-facing clinical teams. The OPH implements and optimizes a strategic, innovative, and data-driven population health operating model across the continuum of care, with the goal of improving health outcomes for our patients and communities.

About this position:

TThe Clinical Referral Coordinator will be responsible for performing chart reviews of patient’s referred to the Office of Population Health multi-disciplinary Cross Functional Care team and identifying the appropriate OPH program for the patient based on a defined referral algorithm. The referral coordinator will also be responsible for ensuring patient’s that are not eligible or meet the criteria for the OPH programs are connected to other resources for further case management assistance.

Who Should Apply:

  • Bachelor’s Degree from an accredited school of nursing.
  • Current registered nurse with licensure in the state of Maryland. Certification in care management preferred.
  • Minimum five (5) years clinical practice. Three (3) years care management or referral coordination preferred. Managed care experience preferred.
