POSITION SUMMARY:

Under the supervision of the project manager, provides services and support to clients and their families to resolve family, social and environmental problems which interfere with participation in viral hepatitis treatment, vocational training, employment and other needed services. Makes referrals to outside agencies; coordinates linkage to care and continuity of care. Serves as a clinical resource in a specialty area. Coordinates improvement in departmental unit and the organizational system to benefit patient care/outcomes. Provides administrative leadership in the absence of the Director of Case Management and Manager of Case Management. Provides clinical oversight and consultation.

Job Duties:

  • Performs case management services for patients with viral hepatitis throughout the period of linkage to viral hepatitis care until patient has been successfully treated/completed viral hepatitis care.
  • Account tracking and invoice reconciliation for Patient Care Fund including Yellow Taxi Cab expenditures, medication co-payments and bus tokens for the viral hepatitis program.
  • Development and implementation of the Viral Hepatitis Case Management Staff Training and Resource Guide as well as hands-on training of new case managers.
  • Outreach linkage to care case management services at community hepatitis C and HIV screening events.
  • Provision of comprehensive HIV case management services for persons linking to viral hepatitis care who also need HIV linkage to care in the John G. Bartlett Specialty Practice.
  • Creation and operationalization of the rapid hep C point of care testing and linkage to care case management program in the John G. Bartlett Specialty Clinic.
  • Community outreach hepatitis C education and case management services for staff and clients at local agencies and addiction centers including but not limited to: Man Alive, Broadway Addiction Center, Bayview Addiction Services, JHH inpatient Case Manager and Social Worker staff in-service,
  • Development, coordination and implementation of the case management walk-in program in the John G. Bartlett Specialty Practice.
  • Development, coordination and implementation of topics for Social Work Grand Rounds sponsored by the Viral Hepatitis Program in the Bartlett Specialty Program.
  • Development and operationalization of patient tracking database and referral management protocol to improve linkage to viral hepatitis care from the JHH ED, JHH inpatient units, Bayview Medical Center and the community.
  • Knowledge for Medicaid, Medicare, PPO’S and HMO’s insurance for linkage to HCV treatment
  • Knowledge of home and community based Services

COMPETENCIES:

Education: Bachelor’s Degree in human services or a related field. Minimum five (5) years relevant experience required.

Knowledge: Strong knowledge of viral hepatitis and HIV disease and management, knowledge of common barriers to care, associated medical and psychosocial problems, and the details of a typical course of viral hepatitis and HIV treatment; knowledge of family systems and parenting skills; knowledge of addiction and varied resources to support engagement and retention in viral hepatitis care, ability to facilitate client support groups, knowledge of medical insurance systems and referral management.

Additional Qualities: Analytical ability to assess client needs, develop associated care plans and provide effective support; good verbal and written communication skills to exchange information and interact effectively with clients and their families, other service providers, community audiences, etc.; excellent presentation skills and organizational skills; Ability to facilitate effective work relationships across agencies.

Empathetic and nonjudgmental; energetic and able to work with high level of independence; high comfort level in the East Baltimore community; willingness to attend Hep C and HIV community testing events, health fairs, educational events to raise awareness, promote health literacy and maximize opportunities for tight linkage to care.

Required Licensure, Certification, Etc.: None

Work Experience: A minimum of five (5) years full-time experience in a human services setting (may include internship experience).
