Johnson County Health & Environment department is looking for a qualified candidate to fill the position of a Program Manager of Population Health/Health Equity. Duties Include:

  • Manages the design, implementation, and evaluation of the Centers for Disease Control and Prevention’s Public Health 3.0 concepts and strategies initiative for the County; chairs the Public Health 3.0 Task Force and ensures collaboration and communication regarding task force throughout the department, the County, and with other community organizations; maintains networks and relationships with community stakeholders in support of public health 3.0.
  • Designs, implements, and evaluates health equity efforts and the expansion of public health capacity; collaborates with stakeholders and collects data related to public health to address local health disparities and identify determinants to health and health equity infrastructure needs of underserved populations within the county; identifies and works with community-based organizations and leaders and engages with various organizations in summarizing, developing, implementing, and evaluating policies, programs, and services affecting the health of marginalized communities county wide.
  • Serves as a member of the department’s executive team and develops, implements, leads and evaluates activities and strategies to support broad-based health equity within the department and County; collaborates and communicates with the quality improvement, strategic planning, workforce development, health equity and other internal teams to assess systemic needs and develop health improvement initiatives; analyzes and interprets policy related to health equity and Public Health 3.0.
  • Manages the work of the Health Equity Council (HEC) to move health equity work forward to ensure internal programs and services, practices, strategies, and procedures are inclusive and implemented with an equity focus; identifies gaps in engaging the community; ensures community engagement and co-production with community.
  • Coordinates and leads Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) efforts and on-going community assessment of needs; directs efforts to collaborate with area health systems on CHA and CHIP processes; contributes to the development of funding sources to support internal and external health policy and equity initiatives and programs.
  • Serves as staff liaison and ex-officio secretary for Public Health Leadership Council (PHLC), including the planning, managing, and coordinating the work of the council.
Job Requirements

Requires a Bachelor's degree in Public/Community Health, Health Education, Public Policy, Urban Planning, Public Administration, Social Welfare, Sociology, Anthropology or a relevant field.

Five (5) years of professional experience performing health-related program coordination, coalition coordination, or system management relevant to the essential functions is required. Grant writing experience is also required.
