General Overview:
JSSA’s Aging in Place department has an opening for a part-time Case Manager. Our Aging in Place department is comprised of experienced professionals who provide full-service care management, counseling for individuals, couples and families and socialization programming. Our care management services include assessments, care coordination, referrals, advocacy and access to appropriate benefits. Our team works closely with contacts in the community, improving service delivery for our clients. The Case Manager would carry a caseload of clients in Montgomery County, Maryland.
Responsibilities:
Provide Care Management to our Holocaust Survivors and Aging in Place clients
Work as part of interdisciplinary team
Provide elder care consultations
Assess needs, create and implement treatment plans
Identify and connect older adults to the resources in the community
Position Qualifications:
Master’s degree in a related field;
Clinical or postgraduate licensure in Social Work
LCSW-C licensure preferred
Experience and enthusiasm working with seniors
Demonstrated experience conducting in home assessments, providing information and referral to eldercare resources, and working with diverse populations
Experience with documentation in an EHR
Independent and reliable transportation
