Duties & Responsibilities:
- Work cooperatively with IL Coordinator in the implementation and ongoing coordination of IL services and activities.
- Complete IL assessments and reports by specified times in accordance to program standards (e.g. System Reports within data base, surveys, etc.)
- Plan and facilitate meetings, individual sessions, group sessions, classes, camps, and activities according to the needs of the Independent Living Youth.
- Liaison with community resources in order to build a pool of available instructors, volunteers, etc. necessary for the implementation of the IL program.
- Interact with IL youth on a one-on-one basis providing individual instruction as deemed necessary per his/her IL plan and living situation to transition to self-sufficiency through exploration of education, career, vocational, employment, daily living skills, fiscal and budgeting, substance abuse and prevention activities.
- Follow up on all necessary activities with Aftercare youth including daily assistance if necessary to ensure all needs are being met including housing, education, employment, as well as, safety and support needs.
- Act as the “first responder” for emergency calls with the aftercare youth.
- Assist youth in acquiring items earned through the stipend program.
- Work with the management staff frequently to develop and oversee services, participate in individual supervision, staff meetings and case consultations.
- Follow guidelines and requirements for Youth Independent Living Services as outlined by OCYF Youth Independent Living Services Guidelines 3130-11-04. This would include but not be limited to: conducting Independent Living assessments and intake processes, coordinating necessary service provision for Youth, and providing follow up care.
- Maintain a flexible daily schedule and be required to work non-traditional work hours and days frequently and respond to phone or physical emergencies on an as needed basis.
- Maintain case records and required documents for client files and database.
- Complete in a timely and quality fashion all documentation and database input as per program requirements but not limited to: tracking aftercare efforts, maintaining a system of tracking all Life Skills training and coordination activities, as well as, creating and completing surveys for the purpose of implementation and improvement of the IL program.
- Transport youth in personal vehicle to youth homes, placement locations, foster homes, IL class sites, etc.
- Develop cooperative relationships with the County’s full range of human services not limited to Mental Health, Drug & Alcohol, educational and vocational, welfare, health care and recreation. Research topics and issues related to IL classes and IL youth needs.
- Participate in court hearings, legal proceedings such as abuse court orders, magistrate court for truancy/fines etc., county meetings, new training, state or county initiatives or requirements as pertaining to Aftercare youth.
- Participate in supervision and staff meetings for work performance, community resources, techniques, peer case reviews. Review, develop skills and knowledge base and any corrective actions needed to perform job.
- Assist in backup and coverage on any Family Resource Specialist (FRS) cases as needed and as directed by supervisor with the mission of improving outcomes for dependent and delinquent youth as per the FRS job description.
- Submit accurate, required client notes within 24 hours of service and complete required reports within assigned timeframes.
- Transport clients utilizing personal vehicle as necessary.
Requirements:
- Master’s or Bachelor’s Degree in social work; criminal justice; psychology; counseling; education; sociology; political science, and a minimum of one (1) year direct care experience or supervised practice in lieu of the experience requirement.
- Valid driver’s license, auto insurance and access to a vehicle.
- Pass current, applicable clearances.
- Pass pre-employment, post-offer drug screening.
- Ability to work non-traditional hours including evenings and weekends.
