POSITION SUMMARY

General Medicine Care Coordinator works within the general medicine care team to support the non-clinical aspects of patient care. The General Medicine Care Coordinator will be responsible for connecting patients to community resources and screening patients for social needs. Some patient follow up with be required, but the majority of interactions will be on a one time basis. The General Medicine Care Coordinator will help the medical team fill in social gaps to help patient care run smoothly.

ESSENTIAL DUTIES AND RESPONSIBILITIES

·        Collaborate with other team members, including providers, nurses, Behavioral Health Consultant, Health Insurance Navigators, CHWs and other staff.

·         Review EHR schedule and collaborate with care team

·         Meet with supervisor regularly to improve program and increase patient engagement in clinic services

·         Ability to use multiple systems to document and check patient data

·         Complete all telephone and face-to-face contacts with patients in a timely and professional manner.

·         Evaluates the client’s eligibility for government entitlement programs (i.e. Medicaid, Medicare, Social Security etc.) and refers them as appropriate.

·         Provides ongoing client referrals to appropriate community resources and services based on client need.

·         Assist patient to schedule medical and social service appointments and attend appointments as needed or requested.

·         Provide assistance with medical and behavioral health referrals to ensure client access and follow-through.

·         Assist with coordination of medical records as needed for care continuity

·         Assist patient in securing transportation to medical and social service appointments.

·         Assist patient in obtaining necessary medications through patient assistance programs and other reduced cost programs.

·         Complete all required documentation and data tracking in a detailed and timely manner.

·         Assist with all other duties assigned.

·         Must have strong PC proficiency and MS Office skills                         

·         Must be able to manage time effectively in order to complete tasks and meet deadlines

·         Must have good interpersonal skills with ability to relate to diverse groups of people and people on all levels

·         Ability to prioritize workload and manage multiple tasks with different deadlines

 

MINIMUM REQUIREMENTS

  • 2 years of case management experience
  • Case management experience with low-income, uninsured populations
  • Experience in navigating health and social service systems

 

PREFERRED REQUIREMENTS

  • Bi-lingual in English and Spanish
  • Bachelor’s degree in social work or a related field

Working Conditions

General working conditions are in a healthcare facility. As such, there is a potential exposure to virus, disease, and infection from patients while performing the duties of this job. May experience traumatic situations including psychiatric traumatized and deceased patients. Travel may be expected to community meetings, client homes, or other agencies.

Physical Demands

While performing duties of this job, employees are regularly required to set, walk, and stand; talk and hear; both in person and by telephone; use hands repetitively to finger, handle, feel, or operate standard office or clinical equipment; reach with hands and arms; and occasionally lift up to 20 pounds.

Mental Demands

While performing the duties of this job, employees are regularly required to use written and oral communication skills; read, analyze, and interpret data, information and documents; analyze and solve non-routine and complex problems; use math and mathematical reasoning; observe and interpret situations; learn and apply new information or skills; perform highly detailed work on multiple, concurrent tasks; work under intensive deadlines with frequent interruptions; and interact with others outside of their department.
