Job Title: ACT Team Lead

Job Classification: Salary Exempt

Reports To: ACT Program Supervisor, Clinical Director of Community Programs

Work Schedule: 1.0 FTE Monday-Friday

Primary Responsibilities:

The Assertive Community Treatment (ACT) Leader is responsible for 50% direct service delivery as a mental health professional for persons served in the ACT program and 50% supervising the comprehensive assessment, treatment planning, and service delivery for each ACT client and ensuring fidelity of the Core Services of an ACT team. Is responsible for overseeing and managing the daily operations of the ACT program, including scheduling and coordination of services, and ensuring that the program adheres to best practices, is fully staffed, and is delivering high-quality services that meets the needs of the program participants. Will work as part of the ACT multidisciplinary treatment team, lead daily team consults, provide training, and care coordination. Will keep accurate records and assist in data collection for the program. Will supervise the ACT team, including hiring, training, and evaluating staff.

Minimum Qualifications:

· Requires a master’s degree in social work, psychology, rehabilitation counseling, nursing, or occupational therapy

· Kansas BSRB licensure preferred (LMSW, LSCSW, LPC, LCPC, LMFT, etc.)

· Minimum of two years of experience providing direct services to individuals with SMI/SED

· Knowledge of evidence-based practices for individuals with SMI/SED

· Demonstrates excellent written and communication skills

· Previous experience in a leadership or supervisory role, with strong skills in team building, conflict resolution, and decision making

· Ability to work independently and manage multiple projects and tasks.

Key Responsibilities:

· Provide leadership, support and direction to the ACT team, which includes the roles of Program Assistant, Case Manager, Substance Use Counselor, Employment and Housing Specialist, Individual and Family Therapist, Peer Support Specialist and Psychiatric Nurse.

· Conduct regular team meetings to discuss client cases, treatment plans, and program goals.

· Ensure that services provided by the ACT team adhere to evidence-based practices and meet the needs of individuals with severe mental illness.

· Review and approve treatment plans, crisis interventions, and discharge plans, ensuring that they are comprehensive, individualized, and goal oriented.

· Monitor client progress and outcomes, making adjustments to treatment plans as necessary to promote recovery and wellness.

· Foster a collaborative team environment that emphasizes communication, teamwork, and mutual respect.

· Coordinate care with other providers within the CCBHC and external agencies, including primary care providers, housing providers, vocational rehabilitation programs, and legal services.

· Develop and maintain positive relationships with community partners, stakeholders, and referral sources to enhance service coordination and promote access to care.

· Ensure compliance with all regulatory requirements, including those set forth by the Substance Abuse and Mental Health Services Administration (SAMHSA) and state licensing agencies.

· Participate in quality improvement activities, data collection, and program evaluation efforts to assess the effectiveness of services and identify areas for improvement.

· Implement best practices and quality improvement initiatives to enhance the delivery of ACT services and improve client outcomes.

Supervisory Responsibilities:

· This is a supervisory position and will be required to hire, train, and evaluate ACT program staff.

Physical Demands:

· Requires extended deskwork at a computer

· frequent movement around the office building and community settings

· regular travel to other office locations or community locations

· Must be able to lift 15 pounds

Job Type: Full-time

Pay: From $73,000.00 per year
