SUMMARY:

Provides overall service coordination, advocacy, services arrangement for consumers and families. Responsible for the development and implementation of the consumer’s Individual Program Plan (IPP), Person Center Plan (PCP), progress reports and the consumer’s annual reviews, who are in traditional and/or Self-Determination Program. May provide follow-up, reassessments and periodic review of consumer’s progress and program effectiveness. May be required to seeks community resources and provide information and referral. May initiates the purchase of vendored services and monitors service delivery. Keeps abreast of state and federal regulations, agency policies and procedures, and guidelines related to consumer services.

DUTIES AND RESPONSIBILITIES:

A. Essential Job Specific Duties:

  1. Manages assigned caseload, meeting and statutory and agency documentation timelines.
  2. Initiates and responds to consumer, family, service provider, and outside agency communications on topics including service eligibility, service standards, methods, generic agency programs and problem solution.
  3. Provides information through a variety of methods.
  4. Works collaboratively with clients, families and others to develop program plans, and monitors program results
  5. Assist clients and families in their development of Person Centered Individual Program Plans to address needs associated with the client’s developmental disabilities.
  6. Provide advocacy services for clients and their families to facilitate access to and use of public and generic resources. Maintain current knowledge of public benefit programs, community service programs and resources.
  7. When necessary, make referrals to agency vendors/service providers to meet client Individual Program Plans objectives and follow agency rules for the request and authorization of Purchase of Service funds.
  8. Ensure that services are provided per the Lanterman Act, Title XVII, Federal and State laws and regulations and Kern Regional Center policies and procedures. Consult with Program Manager and/or appropriate teams, and ensure all generic resources have been exhausted.
  9. Understand Medicaid waiver requirements for IPP goals and objectives that reflect CDER needs. Ensure that all Medicaid waiver documents are completed within specified period.

  11. Attend planning team meetings at schools or primary programs and any other consumer related special meetings or hearings (court, mental health, appeals, etc.)
  12. Prioritizes and responds to urgent needs of clients and their families as required and appropriately documents activities including Special Incident Reports, medical, legal and financial concerns.
  13. May manage client finances, when Kern Regional Center is payee, in collaboration with accounting department, to preserve and allocate resources to meet client’s needs.
  14. Maintain strict adherence to all confidential laws related to client information
  15. Provide timely and responsive customer service, communicating with other respectfully and effectively in person, on the phone, by email or in writing
  16. Work effectively as a member of a team, willing and available to assist colleagues with work assigned to the team member.
  17. Collaborates and consults with clinical services as client needs are identified. Attends clinical team meetings as appropriate.
  18. Collaborate and consults with legal services as client needs are identified. Attends legal team meetings as appropriate. May be required to write legal documents and reports to the courts, as required.
  19. Produces reports as required.

20. May engage in program planning meetings with consumers/families/independent facilitators/financial management services (FMS) providers to develop Individual Program Plans and individual budgets.

21. May function as an Independent Facilitator (IF) and will be responsible for the coordination and monitoring of service delivery and progress per Self Determination Program guidelines

22. Will be responsible to provide coverage as Officer of the Day and complete required documentation and case management assistance as the need arise

23. May be assigned liaison responsibility to monitor client’s well-being and ensure that the residential provider is adhering to standard of care and expectation for a client residing in regional center vendored facility and/or AFHA setting.

24. Performs other like duties as assigned.

B. Other Job Specific Duties:

  1. Maintains current knowledge of public benefits programs, community services and resources.
  2. Keeps designated supervisor informed of service delivery, emerging and urgent issues.
  3. Completes all forms, reports, and types of documentation according to agency standards and directs paperwork to appropriate services in a timely manner.
  4. Maintain respectful and clear communication with supervisor. Expect and accept guidance, constructive criticism and supervision.
  5. Work alternative hours as required.
  6. Attend all meetings, trainings, and conferences as assigned.
  7. Maintains safe and functional work environment.

Requirements

MINIMUM QUALIFICATIONS:

Knowledge of:

  • Microsoft Word, Excel, PowerPoint, Outlook or similar software applications
  • SANDIS
  • Developmental or related disabilities
  • Person-Centered planning process

  • Apply relevant laws, regulations, requirements, and policy standards

  • Maintain adherence to all confidential laws


EDUCATION AND EXPERIENCE:

The following requirements generally demonstrate possession of the minimum requisite knowledge and ability necessary to perform the duties of the position.


  • BA/BS degree preferably human services or human behavioral science such as social work, psychology, sociology, guidance and counseling, criminal, and child development
  • Two (2) years of experience working with the developmentally disable or appropriate experience.
  • Or any combination of education and experience may be considered
  • Bilingual language fluency (Spanish/English) highly desirable.

OTHER REQUIREMENTS

  • Possession of a valid California Driver’s License and State automobile insurance, and acceptable driving record substantiated by a DMV printout.
  • Full use of automobile or ability to provide for independent transportation
  • Must be able to drive to various sites as assigned on a daily basis.

  • Mobility sufficient to visit branch offices, clients’ homes, community services provider’s sites and public meetings

Salary Range $20.59 to $28.97

Starting Range $20.59-$23.84
