Job Description
Responsible for the coordination and implementation of Case Management Strategies pursuant to the patient plan of care. Responsible for discharge planning, resource and utilization management for assignment areas. The Case manager assures that all regulatory guidelines pertaining to patient care coordination and utilization are met.
Experience
Preferred
3 - 5 years: 3-5 year of case management experience preferred.
Education
Required
Associates or better in Nursing
Licenses & Certifications
