Responsibilities:
- Responsibilities include the coordination of care across the continum, including reviewing clinical notes of potential patient referrals.
- Communicating with patients and family members on the outpatient programs.
- Working with third party payers to obtain authorizations and utilization of benefits for specialty outpatient programs.
- Develop community partnerships and guide patients and staff with appropriate community resources.
- Work with the interdisciplinary team to implement safe and appropriate discharge plans.
The Cognitive Rehabilitative Program (CRP) has diversified programming including traditional full and individual, fundamentals, MS, and Dementia programs.
Requirements:
- Master’s degree in Social Work and current /valid New Jersey state licensure in Social Work required (LSW) or a NJ license Registered Nurse with a Bachelor’s degree.
- Previous experience in with case management or related area with neurologic patient population preferred.
