Outreach Worker - DMH IHBTC, Fall River, MA

The Key Program, Inc. is currently seeking a Full-Time Outreach Caseworker to work with children and families living within Southeast MA and will be based out of our Fall River, MA regional office. The Outreach Caseworker works as a member of the Intensive Home-Based Therapeutic Care team to provide tangible support to youth and families referred by the Massachusetts Department of Mental Health (DMH). 

 What you bring...

• **REQUIRED** Bachelor’s degree in a human services-related field (i.e., psychology, sociology, criminal justice, human development, human services, etc.)  AND a minimum of one-year direct care experience (part-time, volunteer or internship).   
• Must be 21 years of age or older  
• Valid state driver’s license and legally registered and insured car for work use  
• Due to the nature of the work and client needs, the Outreach Caseworker position requires a 40-hour work week. All candidates must have the ability to work a flexible schedule including days, evenings and weekend shifts. On-call duties may also be required.

What you will do...

The Outreach Caseworker is responsible for nurturing and supporting family engagement, providing life skills coaching, and assisting youth and families with achieving individualized treatment goals:  

• Provide advocacy in the areas of health, education, vocation, legal and family/foster family intervention;  
• Facilitates individual client and family informal counseling, mediation and support; 
• Coordinates and leads both individual and group recreational activities;
• Participates in the on-call system for crisis intervention, following agency and program policies and procedures;
• Identifies and implements community service resources for clients;  
• Completes case documentation (assessments, daily treatment notes, reports, etc.) in a thorough, professional and timely manner.
