SUMMARY

Provides support to mentally disadvantaged persons who are eligible for and/or participating in a transitional living initiative. Provides needed encouragement, guidance, direction and training to program participants who have taken occupancy in a transitional living complex (Key Manor and/or Key Vista Apartments). Serves as advisor to the Transitional Living Supervisor on matters related to transitional living concerns and needs of program participants.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other Duties may be assigned.

1. In concert with individual candidates established their residence in the transitional living apartment complex suitable t the individual and in keeping with HUD guidelines/directives.

2. Provides training for the candidates and residents in the following areas and periodically monitors programs, etc.

3. hygiene/health

4. medications/health

5. shopping

6. banking/budgeting

7. household management and kitchen maintenance (general cleaning, use of appliances, bed making, laundry)

8. health, safety and personal security.

9. personal identification

10.meal planning/cooking

11. clothing maintenance

12. travel/transportation

Makes referrals to the Transitional Living program.

Gathers information from multiple sources for all significant areas of a person's life including but not limited to:

  1. demographics/personal history
  2. current living circumstances
  3. financial status
  4. health and medical issues
  5. current support services from paid and unpaid sources
  6. personal relationships
  7. current developmental status and skill levels
  8. domestic functioning
  9. recreation/leisure activities
  10. community functioning abilities
  11. current patterns with rhythms of daily life
  12. strengths what a person can do
  13. weaknesses what a person needs to learn
  14. personal goals and desires of the individual
  15. Designs a personalized Transitional Living plan for individuals.
  16. Implements the individualized Transitional Living plan.
  17. Makes adjustments as needed to the personalized and individualized Transitional Living plan.
  18. Performs a variety of tasks that are necessary to ensure a successful Transitional Living placement for the individual.
  19. Ensures the transitional living complex, (key Manor and/or Key Vista Apartments) meets and maintains high standards of cleanliness and general appearance. This is accomplished by: regular and routine inspections of the complex
  20. Ensuring all maintenance issues are put in writing with a suspense date for follow-up and/or completion.
  21. Instilling a sense of ownership in the residents so that they have a strong sense of responsibility for their individual apartments and the apartment complex in general.
  22. Monitors, as needed, the personal security and safety of program participants as well as other living arrangement components.
  23. Discusses with the TLC Supervisor concerns relative to any aspect of the program participant's living arrangements.
  24. Assures that provisions of the Americans with Disabilities Act (ADA) are compiled with particularly those related to a barrier free environment.
  25. Performs other duties as required.

SUPERVISORY RESPONSIBILITIES

Supervises the functions and activities of assigned transitional living support personnel.

QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE

High school graduate or equivalent certification preferred. One year experience working in a job related field or be a certified nursing assistant.

CERTIFICATES, LICENSES, REGISTRATIONS

Valid Florida driver's license with a safe driver rating.

OTHER SKILLS AND ABILITIES

Ability to deal courteously, tactfully and efficiently with program participants, staff members, family members, and the general public.

Emotional stability and maturity with a genuine interest in working with program participants.

Capable of working a flexible hour/day or weekend schedules as needed.

Some managerial or supervisory competencies required.

Knowledge of life safety and infection control protocols.

Willingness to comply with in-service, training or other mandates.

Ability to use good judgment and good organizational skills.

Ability to make prudent decisions.

Willingness and ability to interact with and train the developmentally disabled.

While performing the duties of this job, the employee occasionally works in outside weather conditions. The noise level in the work environment is usually quiet.
