What are the responsibilities and job description for the Night Shift LPN position at Keystone Hospice?
Keystone Hospice, located in Eagle, is currently seeking a Licensed Practical Nurse for our On-Call LPN position (Monday-Thursday). The successful candidate possesses a current LPN license and exhibits an outgoing, professional, and compassionate presence. Our nurses visit patients where they reside, which could include private homes, assisted living facilities, and skilled nursing facilities.
Duties include but are not limited to: answering and responding to incoming calls and requests for assistance, making after-hours visits when needed, obtaining physician orders as required, communicating with the team (including provider, Case Manager, and other team members of the hospice team) any significant findings, issues, patient’s needs, as well as changes in condition or environment, provide education, resources coordination, pain, and symptom management.
