Job Overview:
We are seeking a compassionate and dedicated hospice care Social Worker to join our team.
The ideal candidate will have experience working with individuals with autism, providing medical documentation, and conducting psychotherapy and counseling services. This position offers an opportunity to make a meaningful impact on the lives of those in need.
Duties:
- Provide individual and group counseling sessions for clients with diverse needs
- Conduct behavioral health assessments and develop treatment plans
- family counseling services to support healthy relationships
- Utilize expertise in Christian counseling, addiction counseling, and social work principles
- Maintain accurate and detailed medical records using ICD-10 coding
Requirements:
- Bachelor's or Master's degree in Social Work or related field
- Valid state licensure as a Social Worker
- Proven experience in individual and group counseling
- Knowledge of behavioral health practices and interventions
- Strong communication and interpersonal skills
- Ability to work effectively in a multidisciplinary team environment
