CCBHC is a SAMHSA grant-funded program aimed at transforming community behavioral health systems and providing comprehensive, coordinated behavioral health care, including integrating physical health care. The position works under the guidance of the Chief Medical Officer to conduct population-based prevention and chronic disease management strategies, diagnose and treat individual patients, and educate clinicians.
Primary Responsibilities:

Develop and implement a population health plan for KMHS consumers across the lifespan using population health management health information technology tools, including patient registries, state-of-the-science methods, evidence-based and/or promising practices, approaches, programs, and materials. Conduct interventions, or with Leadership, determine staff, training, or tools needed to conduct intervention and employ strategies. Train clinicians regarding interconnections of physical and behavioral health and interventions improving health outcomes.
- Work with CCBHC Project Director, Evaluator, Quality Improvement Committee, Senior Outpatient Director, Care Team Supervisors and Coordinators, Psychiatric Care Providers, and Primary Care Providers to coordinate excellent client services.

Monitor and track consumer health trends guiding the development of health programs and measuring impact for the agency.

Maintain a reasonable patient panel across the lifespan. Document patients’ histories, including identifying data, chief complaints, illnesses, previous medical or family histories, or psychosocial characteristics. Perform preventative screening, physical examinations, and physiological function tests. Diagnose health conditions by symptoms and health histories, laboratory and diagnostic radiology test results, or other physiological measurements.
- Prescribe and provide health education, evidence-based somatic therapies, nutrition, and general health counseling; administer, dispense, or prescribe natural medicine. Educate patients about health care management.

Oversee pilot care transitions project for consumers exiting 24/7 settings to outpatient care with one lab-documented health indicator for adults and BMI for youth; provide four or more sessions when the patient begins outpatient treatment. Guide population-based care coordinator to carry out project, including monitoring 24/7 discharge date and follow-up within 48 hours, scheduling of naturopathic or other primary care provider visits.

Consult with other health professionals for patient care, client concerns, medication management, and patient referrals. Provide or refer for immunizations and disease screening.
- Monitor and employ HIT tools for patient care, client patterns, and intervention opportunities, including Electronic Health Records, PCHS HIE, i2i population-based health management platform, EDIE emergency department records, or other HIT.

Practice at all times within the scope defined by the State of Washington and within the practitioner’s knowledge, skill, and experience.

Minimum Qualifications:
EDUCATION: Naturopathic Degree (ND) from an accredited institution (AANMC or CNME) or MD/DO with approved Alternative Medicine residency education.
EXPERIENCE: Completion of naturopathic residency or minimum of 3 years of job-related experience or additional Public/Population Health qualifications.
LICENSURE: ND. Licensed to practice medicine in Washington. Specialization in acupuncture welcome but not required. Active and unencumbered DEA license in WA State.
