The State of Vermont has openings for Complex Care Case Managers to provide outreach and onsite case management services to people experiencing homelessness in the General Assistance Emergency Housing Program. Services are provided onsite at hotel locations.
Case Manager Responsibilities:
- Holistic clinical case management, to support health and health outcomes for individuals with a variety of complex health & social needs.
- Setting is in the field and primarily in hotels that shelter participants of the General Assistance Emergency Housing Program. Field based visits with individuals served occur in a variety of settings including apartment, motel and shelter. The position works closely with the Vermont Chronic Care Initiative Team and Field Directors.
- Service delivery through the complex care model (patient engagement tools, care teams, shared care plans), strength-based approach and motivational interviewing skills.
- Provide outreach, screening, facilitation of health and services resources and team-based care.
- Case conferencing and shared care planning
- Duties involve the need for effective communication and working relationships with a diverse group of stakeholders, including the individual, family members and a number of health care disciplines and service providers; and operational integration with various other health care reform initiatives.
- Experience and knowledge of case management, knowledge of community, and community culture are necessary.
- Skill and experience in de-escalation techniques and motivational interviewing.

We are seeking to fill 1 position in Rutland area. Actual onsite locations are in hotels.
Environmental Factors
Duties are performed in a motel, facility, standard office setting and/or in the community. Extensive travel may be required, for which private means of transport must be available. Certain functions may require duty outside of normal work hours. Some resistance or objection to standards may be anticipated, requiring tact and diplomacy to achieve proper action. Duties involve the need for effective communication and working relationships with a diverse group of stakeholders, including the individual, family members and a number of health care disciplines and service providers, and operational integration with various other health care reform initiatives. Critical assessment skills are necessary to promote personal and client safety.
Minimum Qualifications
MSW or equivalent with 1 or more years of experience.
Special Requirements
Reliable means of personal transportation is required for work out of the office or in the community and field-based settings.
