Position Summary

Responsible for maintaining medical records, overall referral process, and Medicaid travel per KCHC policy and HIPAA regulations. Acts as a resource to KCHC Staff for medical record, referral, and Medicaid travel needs. Assures smooth and accurate processes for maintaining, scanning and organizing medical records.

Essential Job Functions

Acts as a member of the KCHC Healthcare Team in the provision of health services to individuals, families, and/or their communities. Works collaboratively with patients and their caregivers to the extent preferred by each patient as well as with other members of the KCHC Healthcare Team to accomplish shared goals within and across settings to achieve coordinated, high-quality care.

  • Maintains medical records per KCHC policy and HIPAA regulations. Ensures strict confidentiality of all patient information, and pays extreme attention to detail to assure accuracy and completeness of work
  • Responds to outgoing medical record requests in an appropriate and prompt manner, ensuring a signed medical records release is present. Transfers medical records to other medical practices when requested to do so by patients.
  • Interacts with hospitals, State of Alaska Department of Social Services, community-based case management services, veteran services, etc. to ensure effective medical records.
  • Documents patient information in an accurate, concise and timely manner.
  • Performs monitoring of the records and medical fax folder for incoming medical records and ensures delivery of the documents to the appropriate designee.
  • Scans and indexes health information into current Kodiak Community Health Center EHR system.
  • Reviews patient’s external medical record and determines appropriate information to be scanned/indexed into EPIC (EHR) system.
  • Organizes data from other various clinical resources in order to correctly identify and provide accurate patient data for the providers.
  • Understands and properly identifies documents to be scanned into EHR based on the clinical guidelines set forth and the approved scan document list.
  • Receives and processes all Medical Records.
  • Files/scans old medical records into EPIC.
  • Files/scans incoming medical records into EPIC.
  • Safeguards all forms (electronic, written and oral) of confidential information as it relates to patients, their families, medical staff and employees. Is aware of and compliant with organizational polices regarding HIPAA, Fraud and Abuse and Conflict of Interest.
  • Results imaging orders ordered by KCHC providers.
  • Assists with the overall referral process including but not limited to: daily monitoring of Epic referral reports, determines appropriate receiving provider/facility as indicated by referring provider, provides appropriate referral documents to receiving provider/facility, submitting prior authorization requests to insurance, facilitates appointment coordination, systematically communicates referral plans to the patient, regularly updates patients electronic medical record (EMR) to reflect pertinent referral details, maintains referral tracking report to assist in patient compliance with referral plan and ensures that the appropriate notes are attached to the patients chart.
  • Assists with scheduling of medical appointments for KCHC patients referred to outside health care providers. Assists and coordinates travel logistics for off island appointments as indicated.
  • Requests travel authorizations for patients with Medicaid.
  • Performs other related duties, as assigned, that support the overall mission and vision of Kodiak Community Health Center.

Essential Job Qualifications

  • Education: High School Diploma/GED required. Associates or Bachelor degree in health related field preferred. Medical terminology training preferred. CPT coding training preferred.
  • Experience: One year previous medical office or ambulatory care clinic experience required. Must possess excellent customer service skills with caring and understanding attitude. Previous experience and proficiency in the use of computers is preferred.

Ability to work independently in a fast-paced, medical office environment with frequent interruptions, public contact, and occasional crisis situations.

Ability to maintain strict confidentiality with sensitive medical information and foster an ethical work environment.

Ability and willingness to carry out responsibilities in accordance with the organization's policies and applicable laws.

  • Attendance: Regular attendance and punctuality is expected and required.
  • English Language: Must be able to read, write, and speak English.
