Job Summary
The Case Manager I is responsible for assisting individuals, eligible under the state plan, in gaining access to needed recovery supports, medical, social, educational, and other services. Case management duties include assessing the individualized needs of an individual, coordinating provision of services, and documenting services and outcomes. The Case Manager I works in collaboration with the individual’s service team to facilitate access to services needed. The Case Manager I will document all services, contacts and referrals made to clients.
Essential Duties and Responsibilities
The qualification for Case Manager I at KVC Behavioral HealthCare includes a bachelor degree and one (1) year experience; the degree must be in the field of human services. In accordance with 907 KAR 17:015, Section 3(3), a targeted case management services provider which provides a service to an enrollee shall not be required to be currently participating in the fee-for-service Medicaid Program.
A targeted case management services provider shall:
- Agree to provide services in compliance with federal and state laws regardless of age, sex, race, creed, religion, national origin, handicap, or disability; and
- Comply with the Americans with Disabilities Act (42 U.S.C. 12101 et seq.) and any amendments to the Act.

Duties and Responsibilities
Provide direct case management services to adults, youth and families
- Provides intake services including completion of Freedom of Choice form, not completed by the clinician, and introduction to case management service delivery on behalf of KVC KY.
- Gather information on individual functioning for the purpose of integrating that information into comprehensive needs assessment
- Complete Individual’s Case Management Needs Assessment in ChartOnline with periodic review and reassessment of Needs.
- Develop the individual’s plan of care that is driven from the needs assessment and individual input on their treatment needs.
- Complete periodic review of plan of care and revise in accordance with changing needs.
- Monitor follow up of individual in receiving care plan services in accordance with care plan.
- Conduct face-to-face monitoring of individual’s receipt of care plan services every 60 days or more frequently as needed.
- Assist in making referrals for individual to gain access to needed recovery, medical, social, education and other service supports.
- Make contacts with non-recipients with the goal of helping the individual increase access to services; identify supports necessary for individual to be enabled to access services, and for information gathering relevant to the individual’s condition and treatment.
- Conduct face-to-face monthly contacts(as frequently as needed with a minimum of two times monthly) with individuals receiving services, in order to accurately assess needs for case management services, to assist in evaluation of progress, and to identify and monitor the management of high risk situations involving safety or placement disruption.
- Conduct telephone and face-to-face contacts (as frequently as needed with a minimum of two times monthly) with the individual and/or an agency on the behalf of the individual.
- Schedule treatment team meetings monthly to include inviting all members of the Service Team, including: CHFS workers, teachers and school counselors, caretakers, other providers of behavioral health services, and natural supports.
- Organize and facilitate case planning conferences.
- Collaborate with other behavioral health professionals, physical health professionals and community partners to deliver integrated care.
- Attend and participate in collaborative team meetings with clients and their families as well as other treatment providers and community partners.
- Schedule case staffings with the Director of Quality Assurance and other relevant agency staff members.
Funding for this position provided through the Cabinet for Health & Family Services with Kentucky State Funds and/or Federal Funds.
