Position Summary

Coordinates and facilitates the development of the Comprehensive Stroke Program through clinical activities to support the Stroke Program through working collaboratively with members of the health care team to design, implement, and measure safe, cost-effective, evidence-based care strategies for stroke patients. Supports the concurrent and retrospective data collection for ongoing certification of the Stroke Center. Reports quality and outcomes measures to medical staff and nursing committees and develops and implements improvement strategies. Maintains current professional knowledge and competencies and contributing to the advancement of the practice of nursing at the unit/system, local, state, and/or national and international level.

    • Standard Work Duties: Stroke Program Coordinator
    • Role models nursing practice by interacting confidentiality with physicians and interprofessional teams, demonstrating flexibility in complex situations, incorporating feedback and input of others, before reaching decisions.
    • Makes rounds on admitted stroke patients to check documentation, utilization of order sets, NIH completion, dysphasia screening, and Core Measures completion. Reports compliance concurrently to nursing staff and retrospectively at nursing and medical committee meeting. Supervises through active participation with clinical staff in the Get with the Guidelines-Stroke (GWTG-Stroke) quality improvement process. Tracks and inputs data.
    • Collaboratively identifies staff learning needs and provides formal and informal information education to nursing, physicians, and others as needs arise. Collaborates with nursing staff as a teacher, coach, and mentor to ensure positive patient outcomes. Collaborates with nursing leadership to identify clinical activities to facilitate and mentor growth in professional nursing practice.
    • Functions as an expert practitioner for the clinical needs of stroke patients directly and indirectly, in collaboration with the Stroke Program Director as primary physician or identified consultant, following approved stroke specific protocols.
    • Collaborates with all disciplines to ensure that key components of stroke accreditation are met and maintained. Contributes to the development of interdisciplinary standards of practice, guidelines of care, and care, and system-level policy. Develops key tools and processes that support the achievement of quality outcomes for the stroke patient population and creates innovative alternative solutions, acting as a change agent to current system design.
    • Engages in clinical and professional leadership activities such as Nursing Shared Governance, hospital-based committees, community, regional and/or national organizations, and specialty-based program development; promotes customer patient experience, engagement, and service excellence by personal example role modeling these behaviors.
    • Participates in research via one or more of the following: data synthesis, research utilization, and translation of research into practice and generation of new investigations. Communicates advances in Nursing Science promotes the use of and utilizes current evidence-based nursing practice necessary for the advancement of patient care and nursing practice.
    • Partner and coordinator of multidisciplinary delivery of care of stroke patients based on evidence-based standards and guidelines across the continuum of care through key behaviors and activities, including, but not limited to, rounding to ensure Best Practices. Influences systems through change theory as a means to transform care to facilitate expertly designed nursing interventions targeted towards achievement of quality, cost-effective, patient/family-focused outcomes.
    • Creates a practice environment that promotes a healthy work environment that drives the empowerment of nursing, enabling nurses to practice to their full scope, potential, and licensure.
    • Champions evidence-based practice, clinical inquiry, and nursing research, supporting continual quality improvements, safety, and achievement of optimal patient outcomes.
    • Leads organization to accreditation as a Joint Commission Comprehensive Stroke Center (CSC). Maintains program accreditation and leads renewal efforts.

  • Knowledge of clinical aspects of care of the Stroke Patients
  • Meets deadlines for data collection and reporting of CSC, GWTG, core measures, and other quality indicators.
  • Assumes responsibility for coordinating day to day activities to maintain accreditation for the Stroke Program while continually advancing stroke care.
  • Ability to communicate effectively with wide range of employees, leaders at various levels, physicians and other customers. Communicates proactively to prevent problems.
  • Develop PowerPoint presentation and reports and present at a variety of meeting to various medical professionals.Uses technology to develop interactive high level reports that communicate data that is relevant to improving process and outcomes.
