Our Social Service Designee (SSD) heroes have a direct impact on the day to day lives of the patients and residents they care for.

Social Service Designee. Responsibilities:

Discharge Planning

Being a key member of the IDT team

Scheduling and leading care plan meetings

Ordering DME

Writing care plans

Educate on community resources.

Provides the setup, encouragement, transportation & direct involvement for activities specific to the resident’s interests.

Perform other duties as required.

Social Service Designee

SSD certificate from a KDADS accredited program

High school diploma or G.E.D.

You can go anywhere and be a SSD, but at Lakepoint Wichita you can be a SSD hero.

Job Types: Full-time, Temporary

Pay: Up to $35,350.00 per year
