As a Health Mentor some of your duties will be to assists InShape members in developing and implementing their personal wellness program. You will provide support and education related to physical fitness, nutrition and wellness, and you will work independently in community settings with individual patients and leading group activities. Also, you may be asked to expand responsibilities during the pandemic to include case management and functional support when determined to be needed to best support the program. Potential to transport patients in your personal vehicle. Qualifications Bachelor#s degree in Health Science, Social Work, Rehabilitation, Psychology, Education, related field or lived experience. Personal Fitness Certification required within 6 months of employment. Valid driver#s license and vehicle required.
