Lakeshore Talent is thrilled to announce an opening for a dynamic Nonprofit Case Manager with one of our esteemed clients, a prominent nonprofit organization in Aurora, CO. This is your chance to make a meaningful impact while enjoying the flexibility of a hybrid work environment!
Location: Aurora, CO (Hybrid)
Contract Duration: 6 months
Pay Rate: $22.50 - $24.00/hour (DOE)
Work Schedule: Predominantly remote with one in-office day per week and 3-10 client visits weekly
About the Role: This position offers a blend of remote work, in-office collaboration, and engaging community interactions. As a Nonprofit Case Manager, you'll manage individuals receiving waivers and State General Fund services. Your responsibilities will include maintaining essential documentation, coordinating interdisciplinary team (IDT) meetings, and ensuring the effective implementation of Service Plans (SP).
Key Responsibilities:
Serve as a subject matter expert for applicable programs and services
Assess the overall needs of an individual and assisting with the exploration of programs, long-term services, and supports, and other community resources to meet assessed needs.
Ensure the timely initiation and implementation of initial eligibility assessments and coordination process to access HCBS waiver services or other state-funded programs.
Complete assessments, support plan development, and coordination process with the Interdisciplinary Team (IDT) and comprehensive care team.
Conduct assessments to determine eligibility for services and supports
Identify individual strengths, needs, concerns, and preferences
Provide care coordination services, including referrals, education, negotiation, and mediation with individuals and external providers/agencies
Complete accurate case documentation and maintain databases
Prioritize workload and meet all required timelines
Schedule, facilitate, document, and follow-up on all required and ad-hoc meetings
Ensure the accuracy and timely entry and distribution of required documentation
Maintain knowledge of relevant state/federal regulations, policies, and procedures
Uphold a professional and ethical manner in all interactions and meet performance quality, customer service, and coordination standards as assigned
Perform other duties as assigned
Requirements:
Bachelor’s Degree or five (5) years of experience in the field of Long-Term Services and Supports (LTSS), including Developmental Disabilities, or a combination of education and relevant experience
Valid Colorado motor vehicle license, proof of insurance, and a reliable vehicle
Clean driving record (no suspensions and no more than two tickets in the past three years; driving requirements may be waived with reliable transportation)
Preferred Qualifications:
One (1) year of experience in developmental disabilities/long-term services and supports
