Van Driver - Lakewood

Principal Responsibilities:

• Provides transportation of residents to and from physician appointments in a timely manner.
• Provides transportation of current or new residents from hospital after discharge.
• Transports medical records for signatures to medical offices.
• Maintains a high level of safety following safety policies and procedures while in Center and
transporting residents.
• Maintains van in good working condition.
• Maintains van in a neat and clean condition at all times.
• Assists in general with Center activities as needed.
ADMINISTRATIVE
• Exchanges information necessary for quality resident care.
• Organizes transportation of residents to complete required appointments based on priority of resident
needs.
• Maintains and assists with documentation as required by company policy.
• Presents written/verbal report to the Administrator for all repairs needed.
• Attends all assigned meetings and in-services
QUALIFICATIONS
• Verbal and written communication in English for business necessity
• State driver’s license without violations
• CPR certification desired
• Nursing home experience preferred
CONSUMER SERVICE
• Presents professional image to consumers through dress, behavior and speech.
• Adheres to company standards for resolving consumer concerns.
• Ensures that all consumer/resident rights are protected.
