At Lambs Farm our mission is helping people with developmental disabilities lead productive, happy lives and connecting with the human spirit in us all. Lambs Farm is an engaging, intentional community, with the public visiting our learning farm, pet adoption center, restaurant, bakery country store, and thrift store, along with our soon to open greenhouse. These businesses create employment readiness experience of the Participants of Lambs Farm, as they pursue employment in the community at their own pace.

The Case Manager position supports Participants of Lambs Farm to identify, plan and achieve their chosen outcomes. The Case Manager position serves as an advocate and a support for a caseload of 12 adult individuals living on our beautiful lakeside community. The Case Manager supports the home life that the 12 individuals share, coordinates with vocational, day service, recreational, and health services departments, ensuring that each Participant's needs are met, and preferences are respected. The Case Manager supports connecting the Participants of the home to community life, both community on our campus, and the greater community beyond. Communicate information to Participants, families, staff, and other stakeholders. The Case Manager position is a Qualified Intellectual Disabilities Professional certified position, that requires a Bachelor's Degree in a social services or health care provider field, and a year of experience working with people with disabilities.

Responsibilities

  • Identify each Participant’s strengths and needs in areas related to chosen outcomes through the use of assessments.
  • Oversee implementation, monitoring, and revision of outcome based services in accordance to the state and federal guidelines.
  • Develop and implement life skill training programs, support, and service plans as determined by the Community
  • Support/Interdisciplinary team in accordance with state and federal guidelines.
  • Assist the Behavioral Analysts in providing input and tracking behavioral programs.
  • Organize participant annual meetings and facilitate as-needed interim meetings.
  • Summarize program data into a monthly report.
  • Provide instruction to Direct Support Persons so they successfully run and document training programs.
  • Act as liaison with guardians, families, ISC and other outside stakeholders.
  • Assist with financial and insurance concerns.
  • Accompany and serve as an advocate at health, psychiatric and counseling appointments.
  • Follow-up on Participant related behavioral incident reports as required by DHS and IDPH.
  • Attend and make case presentations with the Behavioral Analysts as needed to the Behavior Support and Human Rights committees.
  • Provide crisis intervention as necessary.
  • Direct care and assistance with Activities of Daily Living as appropriate.
  • Maintain QIDP status via continuing education courses

Requirements:

  • Meet federal and state requirements for Qualified Intellectual Disabilities Professional (QIDP) and successful completion of approved QIDP orientation course or successful completion of course within 6 months of employment.
  • Good written, verbal and computer skills.
  • Valid driver’s license with a good driving record.
  • Completion within 90 days of employment and continued certification of CPR, First Aid, NCI, Abuse/Neglect and blood borne pathogens.
  • DSP and medication administration certification within 12 months of hire date.
