Job Summary:
The incumbent in this position provides case coordination services for the CRT clients identified with requiring specialized medical management due to complex medical needs, and documents these services according to accepted business and clinical standards and state regulations.

Education/Experience:
Required: Licensed/certified as a nursing assistant/emergency medical technician/practical nurse 3-5 years of medical/medication services in an outpatient setting, preferably with patients who have mental health conditions. Minimum High School diploma/associates degree equivalent
Preferred: Bachelor's Degree with LNA/LPN/EMT certification/licensure
License/Certifications:
Required: Licensed Nursing Assistant; Valid Vermont Driver's License, proof of legally-acceptable insurance, proof of current automobile registration.
