GENERAL SUMMARY
Provides personal care and support services to participants.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- 1. Provides personal care by following care plan.
2. Performs simple household tasks as assigned to assure safe, clean and healthy environment.
3. Responsible for clocking in and out on telephony system
4. Responsible for being on time for participant visits.
5. Reports patient’s conditions and changes to Case Manager/clinical staff.
6. Complies with Agency Policies and Procedures.
7. Complies with county and state regulations.
8. Complies with annual training requirements.
9. Assumes other duties assigned by Case Manager/clinical staff

KNOWLEDGE, EXPERIENCE, LICENSURE REQUIREMENTS:
- High school graduate or GED.
- Completion of a formal personal care training, state certification preferred
- Preferred 6 months of personal care experience
- Ability to work well with other and maintain open communication
- Able to carry out directions effectively and accept supervision
- Valid current driver’s license and necessary insurance coverage as mandated by the state.
PHYSICAL JOB DEMANDS:
1. Ability to physically move, lift or shift 100 pounds.
2. Ability to stand, walk, climb and descend stairs, reach above objects involved in the care of patients.
3. Able to safely operate a motor vehicle.
WORKING CONDITIONS:
- Normal working hours are Monday through Friday 8:00 a.m.-4:30 p.m. and some weekend coverage.
- Aides may have to work Holidays depending on participants need will be communicated from Scheduling Coordinator
3. Maintain a safe driving record and a vehicle in dependable working order for various assignments.
