RN MDS Coordinator

```Job Overview:
We are seeking a dedicated and experienced MDS Coordinator to join our team. As an MDS Coordinator, you will play a crucial role in ensuring accurate and timely completion of Minimum Data Set (MDS) assessments for our patients. Your attention to detail and knowledge of regulatory requirements will contribute to the overall quality of care provided at our facility.

Responsibilities:
- Conduct comprehensive assessments of patients' health status and develop individualized care plans
- Collaborate with interdisciplinary team members to ensure accurate and complete documentation
- Monitor and update MDS assessments as required by regulations and changes in patients' conditions
- Ensure compliance with all state and federal regulations related to MDS assessments
- Coordinate with healthcare providers to obtain necessary medical documentation for accurate coding
- Participate in quality improvement initiatives to enhance the overall care delivery process
- Provide education and training to staff regarding MDS assessment processes and requirements

Qualifications:
- Valid nursing license (RN or LPN)
- Previous experience as an MDS Coordinator or similar role preferred
- In-depth knowledge of MDS assessment processes, Medicare, and Medicaid regulations
- Strong understanding of infection control practices in healthcare settings
- Familiarity with eClinicalWorks or similar electronic medical record systems
- Ability to perform physical examinations and phlebotomize as needed

Job Type: Full-time

Pay: $28.97 - $34.89 per hour

Expected hours: 40 – 55 per week
