LaSalle School, a multi-faceted human service agency in Albany, New York, has open positions serving the Capital District.
We are seeking part time (minimum of 10 hours per week) and per diem (minimum of 5 hours per week) Community Based Service Providers who will be working with youth and families during and after placement to help reduce the length of stay in a residential facility now and to help prevent future out of home placement. Some of the services provided by the Community Based Service Provider could include, but are not limited to:
- After Care Services
- Community-Based Services through Community Psychiatric Supports and Treatment (CPST) and Psychosocial Rehabilitation (PSR) Services
- Children and Family Treatment Support Services (CFTSS) array to include Intensive Interventions, Crisis Avoidance, Intermediate Term Crisis Management, Rehabilitative Psychoeducation, Strengths-Based Service Planning, Rehabilitative supports, Personal and Community Competence, Social and Interpersonal Skills, Daily Living Skills, and Community Integration activities.
This position is seeking part-time and per diem providers. The part-time position requires a minimum of 10 hours per week and the per diem position requires a minimum of 5 hours per week. 80% of the time is client contact. Additionally, this position requires flexibility of work hours (evening and weekends hours are necessary). This position provides services to multiple counties in NYS and will require driving of up to an hour to meet with youth and families. This role also requires these skills to be successful: excellent communication with strong interpersonal skills and computer skills, Able to manage multiple tasks, set priorities, and manage deadlines, excellent clinical skills, including assessment, engagement, and therapy, knowledge of community-based services, and able to work as a team member and effectively use crisis management skills.
Essential Duties and Responsibilities: Provides counseling and supportive services to youth and families in both our Outpatient and Residential Services Programs.
- Collaborate with family and service providers to identify goals and objectives, and establish and implement the Treatment Plan, based on the child and caregiver's needs and preferences.
- Coordinate with the child and caregiver on scheduling and provision of services.
- Complete all required documentation within required time frames.
- Facilitate service provision and attend relevant collaborative meetings as required.
- Share information related to delivery of services with stakeholders as required.
- Provide comments and information to collateral contacts for the purpose of Treatment planning, assist in tracking progress.
- Attend all required training.
- Meet regularly for clinical supervision.
Education and Experience:
Required:
- Bachelor’s degree in human services, behavioral health fields, or another related field.
- Must have a valid and clean NYS driver’s license as determined by the agency.
- Minimum of 2 year of experience in children’s mental health, addiction, and/or foster care/child welfare/juvenile justice.
