What are the responsibilities and job description for the Case Manager position at Lawrence Court Halfway House?
General Overview:
The overall responsibility of the Case Manager is to supervise the case management and treatment process under the direction of the Clinical Director: evaluate the needs of patients, identify services needed, create a plan to provide care, and bear primary responsibility for the coordination of discharge planning activities.
Responsibilities:
- Coordinate client care with the clinical staff as well as with external services.
- Monitor the treatment process to ensure that necessary services are provided to the clients on a timely basis.
- Maintain comprehensive, relevant, legible progress notes.
- Attend clinical staffing meetings and collaborate in the formation of discharge plans, aftercare arrangements, and referrals for clients.
- Facilitate psychosocial and/or therapeutic groups.
- Other program-related case management.
Qualified applicants will be able to demonstrate the following skills:
- Understanding of the disease of addiction and the Chronic Disease Model of Addiction Treatment
- Ability to communicate effectively with clients, staff, and other professionals
- Ability to respond appropriately in crisis situations
- Ability to educate clients and family members about community resources
- Ability to function as an effective member of the therapeutic team
Position Requirements:
- Minimum license/certification required: eligible for ADT
- Preferred license/certification: LMSW/LGSW, LCPC/LGPC, LCADC, CAC-AD
- Experience in a residential treatment preferred
- Experience with benefits, entitlements, discharge planning, linkages to care
- If in recovery, must have at least two years continuous sobriety
