What are the responsibilities and job description for the QMHP position at Leaders for Life, Inc.?
Seeking an individual to work in our Mental Health Skill-building (MHSS), Intensive In Home (IIH) program and Crisis Program. Candidates will be working with adults with serious mental illnesses and co-occurring substance use as well as children with behavioral problems so severe that they are at risk for out of home placement or recently placed back into the home from out of home placement.
Duties include, but are not limited to:
Maintaining a case load of 2 adults (MHSS/Crisis) and 2 children (IIH)
Development and implementation of individualized service plans with on-going documentation of progress
Linking individuals to community resources as needed
Case coordination with agencies
Training individuals on daily living skills
Completing supervision with clinician as well as attending staff meetings and trainings.
Complete and submit weekly progress notes.
Provide individual and family therapeutic sessions and interventions as outlined in the individualized service plan.
Provide on-call crisis intervention services to IIH clients as necessary.
Establish and maintain effective working relationships with other community agency professionals by working cooperatively with these community professionals.
Perform other duties as assigned.
Must have at least a bachelor’s degree in the human services field and at least one year of clinical experience working with children.
Must have or be eligible to apply for QMHP-A and C certification through the VA Board of Counseling
