Leading Home Care, the leading Home and Community-Based Services (HCBS) provider for Medicaid waiver services in Louisiana, is searching for an eager, talented and compassionate Client Care Manager Trainee (CCMT) to be a part of our team.

The Client Care Management Trainee (CCMT) Program was created to provide hands-on experience to candidates who meet our educational qualifications and are interested in starting a career in case management but have little to no experience in healthcare or human services. This is a development role which is a pipeline to the Client Care Manager position. It is intended as a stepping stone and not a terminal position. Once a Client Care Manager role within the region becomes available, the Trainee will be promoted into it. The CCMT Program gives trainees the opportunity to learn about the HCBS industry, program requirements, and client needs before accepting the responsibility of a fully functioning caseload. HCBS companies provide opportunities for recipients to receive personal care services in their home rather than in institutions or nursing homes. These programs serve a variety of population groups including Medicaid recipients, elderly, Veterans with VA benefits, and people with intellectual or developmental disabilities, physical disabilities or other illnesses.

Responsibilities of a CCMT:

  • Spend time shadowing/observing all LHCL departments for a holistic understanding of business operations (Staffing, Nursing, Client Care)
  • Review and become familiar with required deadlines, acronyms, program rules, forms, etc.
  • Observe & participate in Comprehensive Plan of Care (CPOC) planning meetings, client/family relations, communication, & problem solving
  • Assist with creating & implementing client-specific Care Plans based on the State CPOC and with input from the client or their representative, ensuring that clients’ medical, behavioral, and mental health needs are met
  • Observe Caregivers & work planned in-home shifts to gain an understanding of in-home care so that you may later supervise, train, direct, and evaluate Caregivers in regards to all aspects of client care, ensuring Care Plans are followed and that clients' health and well-being are maintained
  • Accompany Client Care Managers to clients' homes for meetings and supervisory visits quarterly, annually, and as needed to remain updated on client/family’s satisfaction with services, health and well-being, living conditions, and progress toward goals
  • Manage care for a small caseload of clients in various programs; act as backup to other CCMs while on vacation or out of office.
  • Assist with conducting regular Supervisory Monthly Report / Quality Assurance phone calls with clients, and other phone contacts as needed
  • Experience an investigation & learn how Client Care Managers document & respond to issues and Critical Incidents
  • Properly document all client communications, care plan or medical changes, visit notes, or incidents.
  • Observe & maintain updated and accurate digital and paper records for each client
  • Communicate with Director, Staffing Coordinators, and HR on a regular basis to discuss any needs or issues regarding clients or employees
  • Follow directives related to Emergency Preparedness & Response in dealing with natural or manmade disasters, assisting with client contacts and support as needed
  • Promote LHCL in the local community within allowed standards and establish positive and professional relationships with external stakeholders
  • Participate in after hours on-call rotation and serve as backup staff working in-home with clients as needed
  • Follow policies, procedures, and guidelines established by LHCL, Medicaid program manuals, and LDH-HCBS licensing standards
  • Model the LHCL Mission, Values, & Leadership Behaviors
  • Additional tasks related to Client Care as needed

Requirements:

  • Bachelor’s degree from an accredited college or university, or in final semester of Bachelor’s degree program (Preferably in Human Services or related field)
  • Compassion and eagerness to serve elderly and disabled individuals
  • Strong time management, communication and organization skills
  • Basic computer skills including Windows & Microsoft Office
  • Able to operate office equipment
  • Basic addition/subtraction and general analytical skills
  • The ability to interact on a professional level with LHCL clients and their families, DSWs and representatives of other community-based organizations.
  • Valid driver's license, auto insurance, vehicle inspection, and reliable personal transportation; willingness to transport clients in personal vehicle
  • Must satisfactorily pass all required and applicable State & Federal eligibility verifications including background, driver's license, and other required screeningsMust be able to read, write, and follow instructions in English
  • Physical requirements include stooping, kneeling, reaching, standing, walking, and climbing stairs. Must be able to occasionally lift up to 50lbs (carrying files/boxes, assisting with client lifts/transfers, etc.)

Due to the nature of providing community-based care for people with developmental disabilities, it is necessary that every employee of LHCL must be capable of assisting our clients and LHCL in areas that may be outside the specific elements of this job description. In addition, due to the ever-changing environment in which we operate, this job description is subject to modification at any time.

Job Type: Full-time

Pay: $15.00 - $17.00 per hour
