Job Duties & Responsibilities:
- Responsible for the entire case management process, including: assessment and evaluation, planning, authorizing, advocacy, monitoring, reviewing documentation for completion.
- Responsible for providing a safe and welcoming environment for our individuals , developing case plans according to the needs of each individual and providing ongoing support to individuals as they work toward achieving goals.
- Conduct initial and ongoing assessments and follow-up visits to each individual.
- Assess our LEAP individuals needs and link them with outside services such as primary medical care, outpatient treatment, government financial assistance etc.
- Maintain all documentation and contacts as required by law, regulation and policy
- Advocate on behalf of LEAP individuals to protect their rights under the law and ensure that they receive those services and benefits to which they are entitled.
- Collaborate with other departments and outside providers to support and advocate for LEAP individuals.
- Respond to crises, including providing de-escalation intervention and support to highly agitated individuals.
- Keep supervisor informed of the status of the caseload and unmet needs in the community.
- Ability to cover evening and weekend shifts with certain caseloads.
Job Qualifications:
- Masters degree in Social Work from an accredited college or university is required.
- 3 years customer service experience, preferably in human services, possess demonstrated commitment to customer service.
- Proficient in crisis intervention techniques and risk assessment.
- Excellent communication and interpersonal skills for effective client engagement and collaboration with interdisciplinary teams.
- Ability to maintain professional boundaries, handle sensitive information, and adhere to ethical guidelines.
- Knowledge of community resources and referral networks related to mental health and behavioral health services.
- Familiarity with relevant legal and regulatory frameworks governing behavioral health practice.
- Demonstrated commitment to ongoing professional development and continuous learning.
Preferred Qualifications:
- Experience in a non-profit.
- Prior experience in case management, social services, and mental health.
