Leveraging Equal Access Program (LEAP) is a 501 (c)(3) nonprofit organization that helps individuals with intellectual and developmental disabilities. Our individual's disabilities range from Downs Syndrome, MR, Traumatic Brain Injury, Autism, in addition to other cognitive and physical disabilities. Our team is creative, extraordinary, dynamic, and committed to serving the developmentally disabled community with positive impacts.

*We are seeking applicants only located in the North Bay Region (Napa, Vallejo, Fairfield, Petaluma, San Rafael, Novato, Vacaville)*


Job Duties & Responsibilities: 

  • Responsible for the entire case management process, including: assessment and evaluation, planning, authorizing, advocacy, monitoring, reviewing documentation for completion. 
  • Responsible for providing a safe and welcoming environment for our individuals , developing case plans according to the needs of each individual and providing ongoing support to individuals as they work toward achieving goals. 
  • Conduct initial and ongoing assessments and follow-up visits to each individual.
  • Assess our LEAP individuals needs and link them with outside services such as primary medical care, outpatient treatment, government financial assistance etc. 
  • Maintain all documentation and contacts as required by law, regulation and policy
  • Advocate on behalf of LEAP individuals  to protect their rights under the law and ensure that they receive those services and benefits to which they are entitled.
  • Collaborate with other departments and outside providers to support and advocate for LEAP individuals. 
  • Respond to crises, including providing de-escalation intervention and support to highly agitated individuals. 
  • Keep supervisor informed of the status of the caseload and unmet needs in the community. 
  • Ability to cover evening and weekend shifts with certain caseloads. 


Job Qualifications:

  • Masters degree in Social Work from an accredited college or university is required. 
  • 3 years customer service experience, preferably in human services, possess demonstrated commitment to customer service. 
  • Proficient in crisis intervention techniques and risk assessment.
  • Excellent communication and interpersonal skills for effective client engagement and collaboration with interdisciplinary teams.
  • Ability to maintain professional boundaries, handle sensitive information, and adhere to ethical guidelines. 
  • Knowledge of community resources and referral networks related to mental health and behavioral health services.
  • Familiarity with relevant legal and regulatory frameworks governing behavioral health practice.
  • Demonstrated commitment to ongoing professional development and continuous learning.


Preferred Qualifications: 

  • Experience in a non-profit. 
  • Prior experience in case management, social services, and mental health.
