This position provides timely evaluation and treatment of eligible individuals experiencing substance abuse through a comprehensive program that includes outreach, early intervention and treatment, education, and alignment of appropriate long-term resources. The advanced practice registered nurse (APRN) provides timely contact with individuals experiencing substance use disorders (SUD) or who have had a recent opioid-related overdose. The APRN coordinates and provides appropriate therapeutic services and community resources necessary to care for individuals seeking treatment for opiate used disorder.
EXAMPLE OF DUTIES
Timely Evaluation and Treatment: Conduct immediate assessments of individuals experiencing substance abuse issues, particularly opioid use disorder. This includes evaluating the severity of the addiction, identifying any co-existing mental health conditions, and determining suitable treatment interventions.Comprehensive Program Development: Contribute to the development and implementation of a comprehensive program to address substance abuse. This includes outreach initiatives, early intervention strategies, individualized treatment options, educational efforts, and the coordination of long-term recovery resources.Coordination of Care: Act as the primary point of contact for individuals seeking treatment for opioid use disorder. Facilitate access to appropriate therapeutic services such as medication-assisted treatment (MAT), counseling, support groups, and other evidence-based interventions.Community Resource Alignment: Collaborate with social services, addiction treatment centers, support groups, mental health professionals, and other community organizations to ensure individuals have access to the necessary resources for long-term recovery.Continuity of Care: Monitor progress, adjust treatment plans as necessary, provide ongoing support and counseling, and facilitate transitions between different levels of care (e.g., from inpatient to outpatient treatment).Performance Improvement Initiatives: Participate in quality assurance and quality improvement initiatives for the program. This includes identifying areas for improvement, providing training and education, and implementing process improvements.
SUPPLEMENTAL INFORMATION
Indoor/outdoor environment. Office work may require sitting for extended periods, working on computers, and making numerous phone calls—the ability to move and lift equipment to 50 lbs. Fieldwork conditions may be noisy, uncomfortable, unkempt, and stressful—occasional exposure to blood or other potentially infectious materials(OPIM). Regularly required to drive county vehicles and to work non-standard business hours, including evenings, weekends, and holidays.
