Job Summary
Summary:
The Intake Specialist, with the assistance of the Post-Acute Nurse Liaisons, is responsible for high volume referral conversion, gathering information related to patients being referred to Lee Health Home Health, The Rehabilitation Hospital, and Lee Health’s Skilled Nursing facilities. Responsible for determining and documenting needs to assist post-acute care teams providing services to the patient. With the assistance of the business office, this position completes and facilitates the entire referral and intake processes and provides support to patients and the other team members across the post-acute care team. Completes daily reports of referrals, admissions, sources, response time, etc along with Post-Acute Nurse Liaison, the business office and the Continuum Relations Representatives. Teamwork, flexibility, and professionalism are central to the success of the team, and are expected in this role.
