Our roots began in 1981 as the Montrose Clinic, with specialization in HIV education, testing, and treatment. Since then, the agency has expanded to >50 clinics in Houston, one in Baytown, two in Beaumont, and one in Deer Park with extensive services that include: Adult primary care, HIV/AIDS care, pediatrics, OB/GYN and maternity, dental, vision and behavioral health. We also service students within KIPP and YES Prep schools. Legacy is committed to driving healthy change in our communities.

Job Description

The Community Health Worker (CHW) is responsible for outreach and health promotion in neighborhoods located near Legacy clinics to increase awareness of developmental screening and health promotion as well as pediatric, dental, and behavioral health services. The CHW provides health information, referral to community resources, linkage to care, and preventative screenings for children ages 0-5. *Position may be partially or fully funded by grant(s).

Essential Functions

  • Provide individual and/or group-level health education and promotion activities in Legacy clinics, home visits, local businesses, agencies, churches, parks, childcare/recreation facilities, schools, etc.
  • Develop and conduct presentations to groups of community members at local community sites.
  • Conduct in-home ECD education, health screenings, and risk assessments within the child's environments.
  • Provide coaching to caregivers on child development, screenings, including milestones, appropriate activities, and games, and how to foster positive adult-child interactions.
  • Promote Legacy service lines and link community members to Legacy as a medical home.
  • Develop, secure, and maintain any equipment or instructional materials needed.
  • Develop, secure, and maintain promotional materials (flyers, brochures, calendars, social media, etc.).
  • Make community referrals to other ECD services to assist in meeting the needs of clients.
  • Document all community education activities and time worked on appropriate forms in a timely and accurate manner in accordance with agency policy.
  • Under direction of leadership, develop and implement procedures for effective community outreach and health education and evaluate effectiveness of class/coaching content.
  • Utilize resources as directed in order to meet grant expectations and goals.
  • Attend training workshops and meetings as required.
  • Other duties as assigned.

Education & Training Experience Requirements

  • High School Diploma or equivalent required
  • Bachelor’s Degree in a related field with a minimum of 1 year of relevant experience preferred. Texas Community Health Worker (CHW) certification preferred but must be obtained and maintained within 1 year of hire.
  • Three (3) to five (5) years of relevant experience is required.
  • Experience providing direct education to clients of diverse cultures and backgrounds in one-on-one and/or group settings.
  • Bilingual Spanish/English.
  • Experience in a healthcare setting.
  • Experience with computers and computer programs
