Great Team- Serving a smaller area with room to grow Ohio and surrounding areaGeneral Summary:Responsible for the coordination of the hospice services program.Relationships:Maintains excellent relationships with all primary constituencies—patients and families, employees, hospice members, and the communities we serve. Coordinates Interdisciplinary Group meetings. Supervises all hospice nurses and home health aides directly or through another staff member. Reports to the Director.Essential Duties and Responsibilities:Performs those tasks and/or functions outlined herein, or others as directed by the Director.Implements and makes recommendations for changes to policies annually.Acts as the Quality Improvement Program monitor, conduct studies and recommend changes.Assists in the development of the annual budget.Implements, interprets, and makes recommendations for changes to goals and objectives.Assists in the recruitment of new hires; interviews new RN, LPN, Home Health Aides, potential hires and recommends whether to hire; recommends hiring or termination of nursing and home health aide staff.Supervises and coordinates pertinent professional contracted employees (either volunteer or paid.)Coordinates and monitors the continuity and quality of care for patients/families for home care component of the program.Monitors and evaluates the implementation of the plan of care. Collaborates with other health care professionals when required.Acts as a resource and provides education, information and/or training for patients/families and interdisciplinary group members for specific care or procedure in home care.Develops and implements programs and training for staff development and orientation.Attends and participates in workshops, seminars, etc., to keep up to date on current regulations and changes in the health care field.Education and Experience:Registered Nurse licensed in our state is required.Baccalaureate Degree in Nursing preferred.Minimum of four years of professional experience in delivering hospice nursing services, which may include at least two years in a supervisory position with a proven track record.Certification in hospice and palliative care nursing (CHPN) preferred.Continuum Care Hospice of Michigan offers competitive wages and a comprehensive benefits package.We provide our residents with compassionate care and quality services to meet all of their needs, both clinical and non-clinical.
